Minor Health & Fitness Form - this form must be completed by minors aged 10-17 for their membership.
All Access Application for 24/7 - if you’re purchasing an annual membership and you are 18+, you may complete this form for 24/7 access. If you want a key card or a key fob instead of using your phone to scan in, you will have to stop by the desk to purchase. Key Cards and Key Fobs are $10 apiece. $20 to replace.
Limited All Access Application, Minor - if you’re part of your family’s annual membership and you are ages 15-17, you may complete this form for limited all access. If you want a key card or a key fob instead of using your phone to scan in, you will have to stop by the desk to purchase. Key Cards and Key Fobs are $10 apiece. $20 to replace.
Walking Track Access Application - if you are signing up for a Walking Track Membership (located at the City Center, 3000 Seneca Industrial Parkway) and you wish to have access outside of public hours, complete this form. If you want a key card or a key fob instead of using your phone to scan in, you will have to stop by the desk to purchase. Key Cards and Key Fobs are $10 apiece. $20 to replace.
